Coleman Professional Services, a nationally recognized not-for-profit provider of
behavioral health and rehabilitation programs, is an organization that is committed to

families, and businesses in our community. We are seeking an individual to provide crisis
intervention, support, information and referral by phone and/or face-to-face in Allen,
Auglaize, and Hardin Counties, Ohio. Responsibilities include completing diagnostic
assessments, prescreens, and answering crisis hotline to provide referrals and
counseling. Individual will be responsible for various reporting, must be able to
be on-call to provide clinical supervision to Access Staff and fill in when needed.

Must have LSW or LPC, a NPI number, an Ohio Medicaid number, and crisis intervention
experience. Must have a clean driving record, able to work flexible hours including
evenings, weekends, and holidays. Prefer someone with their Masters and LISW or
LPCC (with CAQH account).
