About Us
Community Health Professionals (CHP) is a comprehensive, nonprofit home health and hospice organization serving Northwest and West Central Ohio since 1975. The Helping Hands office in Lima was established in 1981.
CHP delivers skilled home health services, hospice, visiting nurses, inpatient hospice, private duty, adult day care, physical therapy, occupational therapy, speech therapy and related services. CHP services are nationally accredited for quality by CHAP and rank in the top 20 percent nationally for patient satisfaction.
